BWM provides our customers with peace of mind, knowing that your private COVID-19 test results will be conducted safely and professionally. BWM is run by medical professionals – Nurse Practitioners and Physicians with decades of international medical experience. When you need reliable results, quickly – count on BWM.

Customers are fully responsible for understanding the requirements of their destination country as well as the airline they are flying with, and for choosing an appropriate collection date and time. BWM’s private lab uses the RT-PCR test OR the Rapid Antigen test as the testing platform, to confirm the presence or absence of SARS-CoV-2 (COVID-19).

  • Results are typically available within 48 hours.
  • Please ensure that you carefully understand and abide by the requirements of your chosen airline and destination country, including any transit points.
  • Once your COVID test sample is run through our approved, accredited lab, you will receive your results by email.

TEST Results

I hereby acknowledge that Blue Water Medical Clinic and its physicians, agents, delegate, employees and Insurers (hereinafter collectively referred to as “BWM”) cannot guarantee you receive results within 48 hours after your sample is taken. As the samples are received and processed by an outside laboratory as they are responsible for the processing and delivery time of the test results.

I Hereby agree I will not hold BWM responsible if I suffer delays or cancellations to my travel plans as a result of the TEST result and/or report, including but not limited to the timeliness accuracy and/or acceptance of the TEST result and/or report.

I hereby agree that I or the undersigned indemnify, defend, and hold harmless BWM from and against any and all claims demands, actions, losses, expenses, damage, liabilities, costs (Including, without limitation, interest, penalties and reasonable attorneys’ fees) as a result of the TEST result and report, including but no limited to the timeliness, accuracy, and/or acceptance of the TEST result and/or report.

TEST Processing

I hereby acknowledge that my TEST sample along with my patient identifying information may be sent to a licensed laboratory in BC. I acknowledge that both BWM and the licensed laboratory will do their best to meet the regulations of the Personal Information Protection Act of BC. My TEST will be conducted and processed by a licensed laboratory. This laboratory is accredited by College of Physicians and Surgeons of BC for COVID 19 Testing. However, I understand that there is no guarantee that the TEST is 100% accurate. I understand I am voluntarily agreeing that agreeing that may have my test and identifying information sent to a licensed laboratory in BC.

Reportable Disease

I hereby acknowledge that if my TEST result is positive, the ordering physician is required by law to report my positive result along with my identifying and contact information to the BC Centre for Disease Control and the local Health Authority.

I hereby agree I will not hold BWM responsible resulting from your Test results being positive, negative or indeterminate for COVID-19. This Includes and is not limited to financial loss, legal ramifications, workplace disruptions, and health complications.

Cancellation Policy

I agree that if I do not give 10 days notice before my appointment. I will be charged for the full amount of the original appointment as a cancellation fee.

Rescheduling policy

I agree that if I do not give 2 days notice before my appointment, I will not be able to reschedule my appointment.

No Refund Policy

I agree that the clinic does not offer a refund policy in the case that the appointment has been cancelled or if the results do not post within the 48 hours window.

The no refund policy applies also in situations where:

  1. Results are returned as NEGATIVE, POSITIVE or INDETERMINATE/INCONCLUSIVE or NEEDS REWORK,

  2. If there is an error in your name or birthday or your lab report

  3. If a catastrophic lab issue including but not limited to: processing errors, shipment delays or error, border delays, lost samples, laboratory delays and lost results,

  4. If we provide evidence we or the outside laboratory emailed you your results, but you did not receive them.

I hereby agree that the relationship and the resolution of any and all disputes arising therefrom between myself and Blue Water Medical shall be governed by and construed in accordance with the laws of the Province of BC.

I hereby acknowledge that the treatment will be performed in the Province of BC and that the Courts of the Province of BC shall have exclusive jurisdiction to entertain any complaint, demand, claim or cause of action, whether based on alleged breach of contract or alleged negligence arising out of the treatment. I hereby agree that if I commence any such legal proceedings they will be only in the Province of BC, and hereby Irrevocably attorn to the exclusive jurisdiction of the Courts of the Province BC.

Acknowledgement

I hereby acknowledge that if I have difficulty in understanding the wording, or the consequences of this consent of this consent, I have been encouraged to seek either the assistance of a translator or to seek legal advice, or both, and have either availed myself of that/those, or I decline to seek such assistance.
